പദ്ധതിയില്‍ അംഗമാകുന്നതിന്
അര്‍ഹതയുള്ളവര്‍
പഞ്ചായത്ത് പ്രദേശത്ത് 5 സെന്റോ അതില്‍ താഴെയോ ഭൂമി സ്വന്തമായിട്ടുള്ള , കുടുംബത്തിലെ 18 നും 59 വയസ്സിനും ഇടയില്‍ പ്രായമുള്ള അംഗത്തിന് ഈ പദ്ധതിയില്‍ ചേരാവുന്നതാണ്.
മുന്‍‌പ് ആബി പദ്ധതിയില്‍ ചേര്‍ന്നവര്‍ ഇപ്പോള്‍ അപേക്ഷിക്കേണ്ടതില്ല

,പഞ്ചായത്ത് സെക്രട്ടറി / വില്ലേജ് ഓഫീസര്‍ / വില്ലേജ് എക്സ്റ്റന്‍ഷന്‍ ഓഫീസര്‍ നല്‍കുന്ന സാക്ഷ്യപത്രം സഹിതം അക്ഷയ സെന്ററില്‍ സമര്‍പ്പിക്കുക

ആരോഗ്യ ഇന്ഷുറന്സ് ഓഫീസില്‍ നിന്നും അപേക്ഷ ഫാറം അക്ഷയ സെന്ററുകളില്‍ എത്തിച്ചു തരും എന്നാണ് അറിയിച്ചിട്ടുള്ളത്. ഈ അപേക്ഷ ഫാറം ആവശ്യമായ
വ്യക്തികള്‍ക്ക് സൗജന്യമായി നല്‍കുക അപേക്ഷ ഇന്ഷുറന്സ് ഓഫീസില്‍ നിന്നും ലഭിച്ചിട്ടില്ലെങ്കില്‍ 1 രൂപ ഫോട്ടോകോപ്പി ചാര്‍ജ് വാങ്ങി ഫാറം നല്‍കുക.

Before you register your
application on-line, please read the information on the website of respective
Passport office (passport.gov.in) carefully, particularly
regarding fee structure, instructions to fill Form No. 1 and How to Apply.
Through this information, you will know the documents required, number of
copies of application form required etc. Apply on-line only if you are sure
that you will be able to produce all the necessary documents on the date of
appointment
Once you register,follow the steps mentioned here-
Step 1: Log into your Passport Seva user account.
Step 2:Select the Apply Passport Online menu option in the left navigation menu.
Step 3: Fill in the required details in the form and submit the form online.
Step 4 (Optional): Upload the required documents using the Upload Document link on the Applicant Home Page. The documents to be uploaded must be self-attested (signed by the applicant) and must be in pdf format.
Step 5: You are required to be present at the Passport Seva Kendra (PSK)/Mini Passport Seva Kendra (Mini PSK) along with original documents for completion of the application submission process. Schedule an appointment with the Passport Seva Kendra (PSK)/ Mini Passport Seva Kendra (Mini PSK) that comes under the jurisdiction of applicant residence.Online appointment is mandatory to walk into PSK for further processing.Only emergency/medical cases may go to PSK without appointment - service will be provided at the discretion of PSK in -charge.

(CHIS)സ്മാര്‍ട്ട് കാര്‍ഡ് പുതുക്കല്‍ മാര്‍ച്ച് മാസത്തില്‍ നടത്തുന്നതാണെന്ന് CHIAK അറിയിച്ചിട്ടുണ്ട്.
TPA ക്കാണ്
ഇതിന്റെ പരിപൂര്‍ണ്ണ ഉത്തരവാദിത്തം .എറണാകുളം
ജില്ലയില്‍ Medsave എന്ന TPA ക്കാണ് ഉത്തരവാദിത്തം. ചില ജില്ലകളിലെ TPA അക്ഷയക്ക് സ്മാര്‍ട്ട് കാര്‍ഡ് പുതുക്കുന്ന ചുമതല തന്നിട്ടുണ്ട്. Medsave അതിന് തയ്യാറല്ലാത്തതിനാല്‍ എറണാകുളം ജില്ലയില്‍ അക്ഷയക്ക് സ്മാര്‍ട്ട് കാര്‍ഡ് പുതുക്കുന്നതുമായോ, പുതിയ
സ്മാര്‍ട്ട് കാര്‍ഡിന്റെ എന്റോള്മെന്റ് സംബന്ധിച്ചോ ചുമതല ഇല്ല. 31/03/2012
വരെ കാലാവധിയുള്ള കാര്‍ഡുകളാണ് ഇപ്പോള്‍ പുതുക്കി നല്‍കുക. നിലവിലുള്ള സ്മാര്‍ട്ട് കാര്‍ഡുമായി കാര്‍ഡിലുള്‍പ്പെട്ട ഒരംഗം
പുതുക്കല്‍ ചെയ്യുന്ന സ്ഥലത്ത്
എത്തിച്ചേര്‍ന്ന് കാര്‍ഡ് പുതുക്കേണ്ടതാണ്.ഓരോ തദ്ദേശസ്ഥാപനത്തിനും അനുവദിച്ച
പുതുക്കല്‍ കേന്ദ്രത്തില്‍ ചെന്നാല്‍ മാത്രമേ ഈ സേവനം
ലഭിക്കുകയുള്ളൂ. APL കാര്‍ഡ് പുതുക്കുന്നതിന് ബ്ലോക്ക് തലത്തില്‍
അനുവദിച്ച APL പുതുക്കല്‍ കേന്ദ്രത്തില്‍ ചെല്ലേണ്ടതാണ്.
നിലവിലുള്ള
സ്മാര്‍ട്ട് കാര്‍ഡുകളുടെ പുതുക്കല്‍ കഴി‍‍‍ഞ്ഞ് അക്ഷയ വഴി ഈ വര്‍ഷം രജിസ്ട്രേഷന് ന‍ടത്തിയവരുടെ ഫോട്ടോ എന്റോള്മെന്റ് നടത്തി പുതിയ കാര്‍ഡ് നല്‍കുന്നതാണ്.
ഇതിനായി രജിസ്ട്രേഷന് ന‍ടത്തിയ സമയത്ത് പേര് നല്‍കിയ ഒരു കുടുബത്തിലെ എല്ലാവരും ഒരുമിച്ച് ,
അക്ഷയ സെന്ററില്‍ നിന്ന് നല്‍കിയ രജിസ്ട്രേഷന് രസീതിയുമായി സ്വന്തം തദ്ദേശസ്ഥാപനത്തിന് അനുവദിച്ച സ്മാര്‍ട്ട് കാര്‍ഡ്
എന്റോള്മെന്റ് കേന്ദ്രത്തില്‍ ചെല്ലേണ്ടതാണ്. നിശ്ചിത ദിവസങ്ങളില്‍ മാത്രമേ ഒരോ തദ്ദേശ സ്ഥാപനത്തിലും
ഇതിനുള്ള സൗകര്യം ഉണ്ടായിരിക്കുകയുള്ളൂ.
